About Everwell
Everwell Community Nursing Pty Ltd is a locally operated, Registered Nurse-led service in Canberra. We provide direct community nursing to individuals and flexible subcontracted nursing to healthcare, aged care, rehabilitation and community care organisations across Canberra and nearby NSW.

Who We Help
Everwell works with participants directly and partners with organisations that need dependable community nursing capacity.
Everwell provides community nursing services to self-managed and plan-managed NDIS participants across Canberra and nearby NSW. We work collaboratively with participants, families, support coordinators, plan managers, supported independent living providers and allied health professionals.
We provide subcontracted community nursing services to Support at Home providers requiring reliable local clinical capacity, continuity of care and responsive nursing support for their clients.
Everwell can support Transition Care Program providers with short-term community nursing following hospital discharge, helping clients recover safely, maintain independence and reduce the risk of avoidable hospital readmission.
We partner with approved DVA and community care providers through subcontracting and brokerage arrangements to deliver eligible nursing services to veterans and war widows in their homes.
We provide community nursing services for people recovering from motor vehicle injuries through approved insurers, rehabilitation providers, case managers and other authorised referral pathways. This includes CTP and ACT Motor Accident Injuries Scheme referrals, subject to referral, funding approval or a letter of authority where required.
We support aged care providers, disability organisations, rehabilitation services, hospitals, discharge teams and community care providers requiring additional nursing capacity or specialised community-based clinical support.
Services Available
Available for NDIS, Support at Home, TCP, DVA and CTP pathways across Canberra and nearby NSW. All services are delivered within the Registered Nurse's scope of practice and are subject to clinical assessment.
Comprehensive continence assessments completed by a Registered Nurse, including assessment of bladder and bowel needs, continence routines, functional considerations, skin integrity risks, support requirements and recommendations for suitable continence products and ongoing management.
Assessment scope
Funding approval and product funding are not guaranteed; outcomes depend on the client's plan and reviewing decision maker.
Registered Nurse support for eligible tube, catheter and drain-related procedures in the community, based on the participant's clinical needs, the nurse's scope of practice, available clinical information and an established management plan.
